fixed inline code blocks that did not get rendered correctly
It appears that there needs to be an empty line before the header so they get rendered correctly
fix links in markdown - no spaces allowed in between [] and ()

The markdown language in github changed over time and made the Readme very hard to read.
I fixed most of the document by changing some of the markdown elements.
