Unfortunately IsStacked=true is not supported on timeproc. I think it might be possible to achieve the same result by adding a HealDoneMultiplier that is limited to the action that triggered Spick and Span (Deep Clean?). This unfortunately is going to require additional testing though, to make sure it works.

The problem with putting a multiplier on Deep Clean is that it only applies a stack of Spick-and-span if Begrimed is already present from Peat Pelt. It is possible to use Deep Clean without having any stacks of Spick-and-span added.

Also, just as a note for Basic Instinct, I listed the heal boost as limited to everything but Spick-and-span, because from my own in-game testing, Basic Instinct isn't applying to the Spick-and-span heal-over-time. I suspect this is a game bug, but it is how it's working in-game currently.

This bug was fixed as of 6.5, Basic Instinct now properly buffs the regen from Spick-and-span. PR updated to reflect this.
