addresses #19 cluster bootstrap SE discrepancy between analytic and bootstrap SE. Turns out
INwas implicitly deduping cluster index vector, so bootstrap samples were too small and didn't have correct coverage.Also fix other cluster-bootstrap-dependent estimators (DuckMundlak and variants).
