Here I've split the marginal effect of additional sensitivity up (A) so that all seropositives are above the axis and all seronegatives below. This means that the "total" lines make no sense on the axes given, so they've been pushed into a new subplot (B) which has the same extents as the cost limit plot (C). Doing this has meant linetype is no longer necessary for differentiating the sensitivities, so the lines are now alpha'ed just like the fill in subplot A and the legend now has less duplication.
