guard against a pre 0.10 json library bug
The BSDs don't have EREMOTEIO, and all we really care about is an error
occurred, so use one that all systems should have.
If json-c is built with the --disable-oldname-compat then bti will not
build.

This fixes it. Obviously it could be more complex but most distros use
the new name now as it's needed for both PHP and PulseAudio.

This fixes the error when using --dry-run, as nothing really fails, so
don't say it did.
It wasn't working anymore, and no one seemed to miss it, so just drop
the code.
No one is using it, so drop it.
change remaining http:// to https://
Thanks to gregor herrmann for pointing these out.

This was only there for identi.ca servers, which is now no longer
supported, so remove this.

Turns out it was never documented, so I wonder if anyone even used it...

Adding oauth_callback=oob signals Twitter to always give us an
out-of-band (PIN code) verifier, even if the "app" has a callback URL
(which cannot be removed).
Request a PIN code during initial app authorization
