Cohort Module 2.0
This is the final release for the Google Summer of Code '16 project Cohort Module Enhancements.
Along with the primary Cohort Module 2.0 the HTML Form Entry Module and REST Services Module are also attached.

I have addressed a lot of issues mentioned in the issue tracker. The main highlights include adding edit functionality for cohorts and all other metadata. Also fixed a variety of tiny but important UI bugs.

Hi @maimoonak @SharonVarghese I have integrated the search widgets completely. I also worked on the locations thing we discussed, fixed the liquibase issues and few other bugs. Hope you can review the module again and provide me feedback. I believe that aside from the HFE part, there are only few other improvements that need to be made in the module (minor bugs/ UI issues)
