This PR updates outdated Polkadot documentation URLs found during a Semrush SEO report and integrity check. The URLs are being migrated from the old Polkadot documentation structure to the new one.
Changes:
- Updated Polkadot documentation URLs from old paths (e.g.,
polkadot-protocol/parachain-basics/) to new paths (e.g.,reference/parachains/) - Added trailing slash to internal template link for consistency
- Removed outdated Substrate documentation reference
