Refactor the method that sets the visibility so that WoW can't turn it back on when you zone to a previous expansion or there are updates to reputations. This also removes the need to hook into PLAYER_ENTERING_WORLD and the need for a delay. We can hide it once at PLAYER_LOGIN and then do nothing unless the player types the /heb command again.

Fix an issue caused by WoW client changes. When you log into the game after each fresh start of the client the UI turns the button back on, even if it's been turned off by the addon. This change turns it back off after a few second delay, since the UI doesn't fire any event that we can hook into.
