Adding CKiD U25 pediatric GFR estimation.
This pull request is designed to add the CKiD U25 equations to the transplantr package. I have attempted to use your package style but I have little experience with R packages. I have not made any changes to documentation but would be willing to help with some guidance.
The CKiD U25 equations are for use in individuals ages 1 to 25, with mild-to-moderate chronic kidney disease (CKD). These equations offer superior estimation of CKD-range GFR across the pediatric and young adult age spectrum and are more appropriate for clinical decision making for adults in the age 18 to 25 interval than the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equations.
