What is being done in this PR?

Add table & column comment support for MySQL Server.

What was discovered while working on it?

func buildTableData gets wrong table information when you have multiple tables with the same name in different databases.
I fixed it.

List the manual test cases you've covered before sending this PR:

I ran make test locally.
