Fix issue #6, #7 and #8

This fixes the issues #6, issue #7 and issue #8 that I ran into when trying this utility with my E-mu ESi-32 unit.

A heads-up that the patch to midi.c I did was poorly tested. The non block mode should be updated otherwise the utility doesn't work properly because it should block later.

After opening the device, these calls should be made:

    snd_rawmidi_nonblock( handle_in, 0 );
    snd_rawmidi_nonblock( handle_out, 0 );
