Fix inconsistent tab order between View and Edit modals across medical subsystems
Edit forms (Documents → Notes) in five subsystems: Medications, Allergies, Conditions, Procedures,
and Immunizations. Updated all Edit forms to match the View modal order (Notes before Documents).
