This change addresses multiple runtime and testing errors in the application. It ensures that environment variables are correctly loaded before any other modules, provides an in-memory database for testing (avoiding connection errors when MongoDB is not running locally), and fixes issues with missing session secrets and geocoder configurations. It also improves testability by disabling CSRF protection in test mode unless specifically requested.

- Moved dotenv.config() to the top of index.js to ensure environment variables are available early.
