Summary: Remove duplicate PR template filename that differs only by case (Windows filesystems are case-insensitive), which was causing “modified file” noise after clone/pull on Windows.
Note: CI is currently failing due to an existing YAML syntax error in .github/workflows/ci.yml (line 2). This PR does not modify CI.
