Changed instruction to reflect creating index.md files: Create a new markdown file in `app/[service-name]/YYYY/MM/[post-name]/index.md` Should this also be changed on line 54?

@veroje Well spotted, and yep, line 54 needs changing too - the images are now in the same folder as the post content.
Alternatively we could reduce the amount of detailed guidance in the README and link out to https://design-history.prevention-services.nhs.uk/guide/ instead? (To avoid having to remember to update 2 places?)
