Description: This PR updates all dependencies that can be reasonably updated. See DEPENDENCY-NOTES.md for exceptions. As part of this PR, I've also bumped our Node to v22.
Testing Instructions: The site should continue to work as expected. Due to changes in SUSHI dependency load order, the extensions package should now be loaded last (after FHIR core).
