1. Update offset param for paging.
   According to the latest Affiliation Retrieval API and Author Retrieval API, the offset for paging is startref, not start. The latter will always get the FIRST page.
2. Fix corner case for paging.

The maintainer updated this project in March, but only added a TODO.

I don't know if Elsevier would like to maintain this project or not :(

I'll send an support ticket to Elsevier to check it.

At this point, I just go this page, I use the endpoints I want through Swagger.
I take the generated CURL url and then I use curl-to-python converter and use it as python code.
It works like a charm.
