Proposal: Markdown AST transformer API

Description

Most MkDocs extensions and plugins are Markdown transformations. Once we move to CommonMark, we'll have an AST to work with, so we should also expose a set of utility functions to work with those ASTs, so module authors can easily provide new functionality. The following sub issues can all be implemented with AST transforms.
