support discourse custom fields

Discourse creates custom fields as user_field_1, user_field_2 and so on.

If a discourse admin creates a user field in their admin, the subdomain field will get the next one in order.

for our purposes, it's user_field_1 but this plugin would need to have an admin view of which field is the one which holds the subdomain.
