duplicate tags

It looks like there are many duplicate "tag"s in the final output of the annual report. Of 16504 tags (pulled from DOGM this afternoon), 2012 were duplicates.

I changed the tag from "API_WellBore_Formation" to "API_WellBore_Formation_FieldName". With still 16504 total tags, now only 598 are duplicates

I added "Operator" to the tag as well, because there were several instances of the same company with slightly different names (for instance, one was X INC. and the other was X LLC)
