REFERENCES is missing in table of contents

I prepared my dissertation using this template, and grad college gave the following feedback:

2. On the Table of Contents, please add "REFERENCES" and the page number 138.

Adding the following before \bibliographystyle can fix the problem:

\addcontentsline{toc}{chapter}{REFERENCES}
