Can you pass in a label parameter?

I was hoping to put a label next to the form inputs versus just a placeholder. Placeholder works great when the field is empty, but is less useful after the user has populated the field.

This can be partially solved with pull request: #38

Though the current implementation will place labels under input, but this branch also includes the ability to add custom CSS classes an input.
