Updates to 'trial details' dialog

As requested by a user:

• Add a text field for 'Field ID': because a database location may represent many fields that should be distinguished at the trial level
• Allow multiple harvest dates in 'Harvest Date' field: useful for ratooning or perennial crops
• Add an option to toggle a calculation of 'Days after Planting (DAP)' and 'Days after last harvest (DAH)' on and off.

User also suggests making leaf icon for trials on Manage/Field Trials page dynamic, such that when Trial Status is updated, color or shape of icon is changed. Ie: active trials are green and completed trials are black?
