Ask the user to confirm the change outputs if there are no other outputs

**Feature description**

At this moment, when signing a transaction, the user is not asked to confirm any outputs with a correctly set `address_index` property. If all outputs have the `address_index` property set to valid internal addresses, the user is not asked for confirmation at all. This should not happen, the user should always confirm any signing operation.
