Setuptools installer

Should consider switching to `setuptools` or `distutils` for the installation; this will also make it easier to pull in any dependencies if necessary in the future. Can still wrap in a Makefile.

This would also let us switch to using _entry points_, rather than a `#!`, since that will not work on Windows. It also solves the problem of cross-platform paths for the install.
