Show topic URLs on management pages

A user has suggested that it would be helpful for `hopauth/manage_topic` to show the topic's `kafka://` URL to make it easy to know what to plug into the CLI client. This should be easy to display if scimma-admin is given a setting/configuration parameter for the base Kafka broker URL.
