Thanks for reporting — to diagnose this I need a bit more to go on. "Tool call interrupted" could mean a few different things and the fix depends on which one it is.

The most likely causes:

1. Running an old version — if you're pre-1.22.0, every turn replays your entire conversation history into a fresh session. As the history grows, tool calls start failing. It'll happen "always" because each turn makes it worse. Check your version and if it's outdated, rebuild/upgrade — this is the most common cause of consistent tool call failures we've seen.

2. Reverse proxy or network timeout — Meridian sends a keep-alive ping every 15 seconds during streaming. If you're running behind nginx, Cloudflare, or any load balancer with a shorter idle timeout, the connection gets cut mid-tool-execution and the call just dies. If you're in Docker, check whether there's anything between the client and the proxy that might be closing idle connections.

3. External MCP tools in internal mode — if you're using Playwright, Exa, or any non-standard MCP tools, those don't execute through Meridian's internal mode (it only provides 6 standard file system tools). The tool call appears to start then goes nowhere. Fix: run with MERIDIAN_PASSTHROUGH=1 so OpenCode handles tool execution.

4. Passthrough mode on an old version — the streaming path for tool_use in passthrough mode had a bug that was fixed in v1.22.2. If you're on an older version in passthrough mode, upgrade.

To help narrow it down, can you share:

• Your Meridian version (package.json or meridian --version)
• Whether you're using passthrough mode (MERIDIAN_PASSTHROUGH=1) or not
• A proxy log line from when it happens (the [PROXY] line)
• What the interruption looks like — does OpenCode show an error, does the stream just stop, does it hang?
