Dublicate Entry when saving

Session: SQLSTATE[23000]: Integrity constraint violation: 1062 Duplicate entry '1008-122563' for key 'PRIMARY'

Sometimes when I save the DynRole I get this.

I tried to define the "Where can add pages" filter. But when looking in the DB table there's no entries. And also it's not working, say the user can't add pages where I defined.

You mentioned you only get that error sometimes. Any factors you can think of that affect when it does and doesn't appear?

I'm using this again in PW 2.6.1 and kinda dependent of this module to set view permission for an intranet.

I still get that error SQLSTATE[23000]: Integrity constraint violation: 1062 Duplicate entry '3342-3400' for key 'PRIMARY'

I have Multiplier installed and using that (same as in screenshot).

I have added and removed various entries to figure out what and how it works best. Now I get this error everytime I add a entry with a "template" selector and save.
