Feature request: ON/OFF switch for caption and source #34

What a great tool you created, thanks!
I used it for several tables, and so far, I never needed the caption in the header and source in the footer. It's easy to manually remove it from the generated code, but it would be even easier if it wasn't added to the code at all. Removing Hed/Dek/Source from the preview before generating the code doesn't work, because it still adds empty rows.
It would be great of you could add toggles to enable/disable the caption/source, just like the zebra striping.
