1. PagerDutyNotificationPlugin URL is incorrect.

This is 404

• written in https://github.com/rundeck/anvils-demo/blob/master/install-rundeck.sh#L87
• The bad URL is https://raw.githubusercontent.com/rundeck-plugins/pagerduty-notification/master/src/PagerDutyNotification.groovy

correct one ( probably)

• https://raw.githubusercontent.com/rundeck-plugins/pagerduty-notification/master/src/main/groovy/com/rundeck/plugins/PagerDutyNotificationPlugin.groovy

2. The above newer plugin causes errors
