Improved volume behavior above 100% for per-app volume control

When I tried to increase the volume of chrome, it don't work. I'm using my AirPods now.
Plus, I think 200% might not enough, 400%?

I checked this in debug mode, and sure enough—the boost mode (200%) currently provides a gain of only +3 dB. I corrected this, and as can be seen from the formula 20 log10(2) ≈ +6.0 dB, it now provides a gain of +6 dB—which is exactly as it should be.
