W/ users, decide how to implement arrivals

Several options:
1.) as part of registration process, allow patients to select location, but assign a date/time (most prescriptive)
2.) as part of registration process, allow patients to select location, but select a date/time
3.) when patients arrive at location, have patient text a location specific number which in turn assigns them a number (like picking a number in a check out lane)
4.) a combination of 1 or 2 and 3

Note that the state of the first two is really "scheduled" versus the 3rd and 4th which are "arrived".
