Wire up the existing insurance schema into full gameplay.

Summary:
- Implement payment of premiums at Police HQ/Fed.
- Enforce waiting periods and coverage rules.
- Implement fraud detection and penalties (alignment hit, fines, bans).
Acceptance criteria:
- Commands to buy/cancel insurance.
- Payout/claim flow integrated with ship loss events.
- Fraud cases logged and penalised according to rules.
