replace .state.json task tracking with github issues

Context

noman currently tracks checklist items in .state.json.
Tasks should be tracked via GitHub Issues instead.

Acceptance

• Remove item-level tracking from .state.json
• .state.json only keeps minimal machine state (cycle, tier)
• Checklist items map to GitHub Issues
