Instructor edit permissions missing + Team Formation icon & label different links

This is for Ervin Teng who on whiteboard is a course instructor:

1. Clicking on any left nav entry works, but Ervin doe not have edit permission on the destination page
   (e.g. http://whiteboard.sv.cmu.edu/pages/Lab1)

2. Course tools: clicking manage assignments does not take him to the destination page. Instead he gets "You don't have permission to index assignment."

3. Same situation for clicking Curriculum Navigation.

For Ervin and for me: Both the Team Formation icon and label have different destination links.
