missing conventions

@drammock should we add these symbols to the not-in-the-IPA table?

• derhoticized
• <ᴅ> U+1D05 LATIN LETTER SMALL CAPITAL D
• <ᶑ> U+1D91 LATIN SMALL LETTER D WITH HOOK AND TAIL
• <ᶣ> U+1DA3 MODIFIER LETTER SMALL TURNED H

There might be a few more... I'll have a look.

np. btw where did you get "derhoticized" from? i don't see it in any inventories (phonemes or allophones).

huh. not sure. I thought it was on the official chart but I don't see it
there. Probably I got it from ishida: https://r12a.github.io/pickers/ipa/
I guess we could ask where he got it from...

if it's not in any of our inventories we can drop it from the docs (and the
fledgling "canonical ordering" and "building feature vectors
programmatically" scripts). On the other hand, it is bad if there are
codepoints in the phonemes that aren't in those scripts, but codepoints in
the scripts that aren't in any of the phonemes doesn't actually cause any
problems. So I'd say fine to remove, but call it a low priority.

Another thing to note in the conventions is the non-standard IPA use of ȵ (lamino-pre-palatals) in the ER source for Australian languages. I should also make sure that these don't conflict with any existing segment conventions for similar sounds.
