Only one Task shows templates

I tried this with:

Environment:
Redmine version 2.3.1.stable
Ruby version 1.9.3 (x86_64-darwin11.4.2)
Rails version 3.2.13
Environment production
Database adapter Mysql2
Redmine plugins:
redmine_issue_templates 0.0.5

Only seems to show the template selector for one task; the last one created. The other tasks have templates defined, but when I try to add an issue using the other tasks, no selector is shown.

Environment:
Redmine version 2.4.3.stable
Ruby version 2.0.0-p353 (2013-11-22) [x86_64-linux]
Rails version 3.2.16
Environment production
Database adapter Mysql2
redmine_issue_templates 0.0.5

Can confirm this problem.
