Create different fields for metadata before uploading to Database

## Goal ##
To add new fields for metadata for dataset to be uploaded.

## Description ##
As of now only the author, title and tags can be mentioned with the data to be uploaded.
This issue aims to add new fields such as date created, Institution affiliated etc.

## Implementation ##

Add new forms for data to be input and alter schema of data being uploaded to textiledb
