time range reminder fires many times 

Reported from a tester with an iPad:

There’s a bug in the time range reminder. It fired, when I hit OK the alert immediately popped up again over 100 + times wouldn't close down, took over my notification banner on my iPad and then froze my iPad (VERY ANNOYING) (occurred mainly with the Intake Survey, I had to delete the app to make it stop).
