Support for yarn v2 pnp

Yeah. How do I configure the node executable that runs the .vscode-test.mjs? We’d need to to either

• run this file with to yarn node
• or with node --require ./.pnp.cjs --loader ./.pnp.loader.mjs

runtimeExecutable in the launch configuration says it’s supposed to point to VS Code, not node, so that’s not it.

the fact that “extensionHost”’s options are for some reason mostly undocumented doesn’t make this easier.
