Incorporate 2024 rules as canonical

Describe the bug
With the 2024 edition of the PHB, Simple5e is anachronstic out of the box. It should reflect the most up to date system rules and terminology.

For example

• Many class resources now replenish partially on a short rest, but Custom Counters and the rest macros can't accommodate that.
• There is a new 'Bloodied' state for creatures under half health.
• All Hit Die replenish on a long rest instead of half.
• "Use an action" has been renamed "Utilize".
• Exhaustion rules have been simplified.
