Update OpenMRS documentation to link to relevant CHT <> OpenMRS interoperability documentation.

Depends on medic/cht-docs#1657 and #115 to be merged.

Community Health Toolkit (CHT) & Medic: See the CHT Integration Guide for OpenMRS Additionally, Medic has built an interoperability layer for the CHT based on the OpenHIE architecture: here. CHT has been integrated with OpenMRS in unique scenarios at peer to peer level based on unique requirements. Community Orgs with known integrations: Partners in Health; KenyaEMR/Palladium (eCHIS)

Community Health Toolkit (CHT): The CHT can exchange data with OpenMRS in both integration (see the CHT Integration Guide for OpenMRS) and OpenHIE architecture-based interoperability (see the CHT Interoperability Guide for OpenMRS) scenarios. Community Orgs with known integrations: Partners in Health; KenyaEMR/Palladium (eCHIS). Community Orgs with CHT <> OpenMRS interoperability initiatives: SunyaEK.
