Improve the CHT Implementation documentation

We would like to restructure the implementation section of the doc site to explain in simple terms what a successful CHT implementation enatails and the steps to get you there.

Here is the proposed outline of the CHT implementation deployment process

• Project conceptualization
• Planning and budgeting
• Implementation, this section will include tool design, technical set up and configuration of the workflows, deployment and training
• Maintaining and sustaining CHT implementations.
