provide installation docs

The installation process has grown into quite a hurdle for non-developers.  Several steps should be documented:
- CouchDB installation (with links for Windows users)
- CouchDB database creation
- initial setup (copy `example.yaml` to `config.yaml`;  provide a quickstart script?)
- how do modules work?
