File upload sequence non-intuitive

When uploading a new file, the workflow is fairly non-intuitive. In particular, the dialog that appears after you click "upload", with boxes for "Upload Details", etc.

In particular:

1. What sort of details should I provide? Is it required? What if I'm batch-uploading multiple files, how will this be used/displayed?
2. Operating System: Is this required? What is it used for? Why is the default Mac?
3. What does "Revision" mean? What is it used for?
4. What is 1/2? [I can guess that this means there's a second screen, but it's not completely apparent.]

Also, the progression buttons are not completely intuitive. I assume clicking "Contact Info" will take me to a second required screen. "Next" would be more standard.

Patch Commited and uploade to beta site. I changed default Operating System to PC (although, I might change it back. We will see), changed Revision to "Revised File", and changed Details to "Batch Details"

Take a look - is this more intuitive? What else should I change?

1. As a user, I'm still not exactly sure what these fields are used for. Why do I need to specify Operating System? Is that what OS I'm using, or where the file was created? Would things break if I mis-categorized? Unless there are good answers to these questions, I'd remove the dialog.
2. Revised file doesn't make sense to me still, but maybe it would after I used the system. Is this saying I'm uploading a file that's a new version of a previous file? If so, can the software infer that?
3. Minor, but maybe consider making the "Batch Details" box smaller, depending on what you expect people to write. If you expect a paragraph or two on each commit, then keep it. But if you expect a twitter-size commit message, i.e. "Checking in powerpoint deck for Project Bananaphone", then it could be smaller.
