Add column information to exceptions

Currently we get exceptions like:
java.lang.NullPointerException: Tried to extract a null value without an OptionExtractor

which don't indicate the column causing the problem this can make problems harder to solve than they should be
In fact any information about the extraction - column/field/extractor name
