[BUG] - Error when trying to submit form (calculator)

I got this error on the latest version of ES Forms:

<br/><b>Warning</b>:  Undefined array key "activity_type" in <b>/home/infobip/releases/current/wp-content/plugins/eightshift-forms/src/Security/Security.php</b> on line <b>99</b><br/>

This is not existent on the previous versions, so I'm not really sure what the issue is (config or something else).

You need to deactivate/activate plugin to install database for new security feature. @dingo-d

That did the trick. I did get

The plugin generated 476 characters of unexpected output during activation. If you notice “headers already sent” messages, problems with syndication feeds or other issues, try deactivating or removing this plugin.

But it activated fine so I guess it's nothing to worry about.

Hi! We updated to v 8.14.4, and I got the same message again.

The form will send the email, but he got the error at the top and the thank you page won't be sent.

I tried deactivating and activating the plugin but it didn't help.
