Kensa doesn't send several documented fields during provision check

Kensa doesn't send the "log_input_url" or the "uuid" during the provision check which is documented here: https://devcenter.heroku.com/articles/add-on-provider-api#provision

The documentation lists several deprecated fields however it lists the uuid as the only reliably unique field.

#82 is already addressing some of these new fields and changes, though some may need further attention.
