high-k extrapolation of CAMB transfer function not working

The extrapolate_with_eh option for the CAMB transfer function doesn't seem to be doing anything at the moment. Also, there's no options to set the maximum k higher.

This seems to be a problem with the form itself not saving the checkbox value. Instead of trying to fix it in this repo, I assume it will be fixed in the SPA version, which will hopefully go live soon.
