feat(telemetry): add cache hit rate metric across all cache systems

Add recordCacheHit() helper that emits a cache.hit_rate distribution
metric (0 for miss, 100 for hit) with a cache_name attribute. This
enables computing cache hit rates as avg(value,cache.hit_rate,
distribution,none) grouped by cache_name on the tracemetrics dashboard
— working around Sentry's lack of division/formula support in widgets.

Instrumented caches:
- dsn: getCachedDsn() single-DSN lookup
- dsn-detection: getCachedDetection() full detection with mtime validation
- project: getByKey() project info lookup
- region: getOrgRegion() org-to-region mapping
- project-root: getCachedProjectRoot() cwd-to-root mapping
- http: getCachedResponse() HTTP response cache (alongside existing cache.hit span attribute)

Updated dashboard 'Cache Hit Rate (%)' widget to use tracemetrics
dataset with avg(value,cache.hit_rate,distribution,none) grouped by
cache_name.
